Having been informed of the organization of QYF Little Dribblers Basketball, I/we do assume ALL risks and hazards incidental to the activities, transportation to and from activities, the sponsors, or any of the supervisors appointed by them. I/we likewise release from responsibility any person transporting my/our child to or from the activities.

I/we further understand that every effort will be made to maintain sportsmanship and good conduct at all activities, games and practices. At no time will a parent or spectator be allowed on the basketball court to criticize a referee or a coach. QLD will not tolerate disturbances or abusive language. One warning will be given; upon the second offense, the person will be asked to leave the activity. A third offense will result in being completely banned from any further games until an understanding is reached between the offender and the QLD. I/we assume responsibility for ourselves and our children in regard to the above and will do everything within our power to see that good sportsmanship and conduct is carried out at all basketball events. I/we understand that each player will be guaranteed to play at least 2 (two) six-minute quarters in each game of the regular season.

I/we further understand that at least one parent of each player will be required to work at least one Saturday, either at the concession stand, the gate or the scorekeeper’s table during the regular season. A schedule will be made and the parent will be notified by the child’s coach of the date/time to work. If the parent is unable to work, I/we understand that it is our responsibility to find a replacement. (All workers must be at least 17 years of age.)

I/we further understand that the uniform issued to each player is the property of QLD and must be returned at the end of the season, unless otherwise specified.

I/we understand that we must furnish a certified copy of the above-named player’s birth certificate upon request. QLD reserves the right to reject any application.

I/we understand that all players must attend Quitman Elementary School or reside in the Quitman Independent School District in order to participate in QLD. I/we further understand that the child must be within the age brackets stated within the Little Dribblers National organization in order to be eligible for All-Stars.

**No players will be allowed to sign up nor will refunds be given after teams have been chosen.
